Heart Health SweepstakesOFFICIAL RULES
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED.
1. HOW TO ENTER: Participants can enter the Rite Aid®/Nature Made® Heart Health Sweepstakes (“Sweepstakes) beginning at 12:01 AM PST on 2/01/08 in one of two (2) ways: (i) On-Line Entry: You may enter by logging on to http://riteaid.naturemade.com clicking on the Sweepstakes link and completely filling out and submitting the Sweepstakes entry form. All on-line entries must be made by 11:59:59 pm Pacific Standard Time (PST) on 04/01/08. Limit one entry per person. Limit of one prize per person. Entries submitted will be deemed to have been submitted by the authorized account holder of the e-mail address from which the entry is made. An authorized account holder is defined as the natural person who is assigned to an e-mail address by an internet access provider, online service provider, or other organization that is responsible for assigning e-mail addresses for the domain name associated with the submitted e-mail address. All Internet entries must be received and recorded by Pharmavite LLC’s (“Pharmavite”) computer (the official Promotion time keeping device) by 11:59 PM PST on 04/01/08; Or (ii) Mail-in Entry: You may also enter the Sweepstakes by hand printing your name, birthday (mm/dd/yy), address (no P.O. Boxes), and (optional) daytime and evening phone numbers on a 3”x5” piece of paper or card, and mailing the entry to: “Rite Aid® / Nature Made® Heart Health Sweepstakes”, Dept. 5525, P.O. Box 5008, Stacy, MN 55078-5008. All mail-in entries must be postmarked by 04/01/08, and received by 04/15/08 to be eligible for the drawing. Each mail-in entry provides one (1) Sweepstakes entry. Limit one (1) entry per stamped outer envelope and limit of one entry per person. Limit of one prize per person.
Pharmavite and their respective affiliates and agencies are not responsible for early, late or undelivered entries. No automated entries will be accepted. Also, entries by macro, both or other similar automated means are not allowed. All entries become the property of Pharmavite and will not be returned.
2. ELIGIBILITY: By participating in the Sweepstakes, entrants agree to be bound by these Official Rules. Sweepstakes is open only to legal residents of the 50 United States and District of Columbia who are 19 years of age or older at the time of entry. Employees of Rite Aid Corporation, Pharmavite LLC, Promotion Mailing Center, Inc., their affiliates, subsidiaries, advertising and promotion agencies (collectively, “Sweepstakes Parties”) and the immediate family members (spouses, parents, children and siblings and their respective spouses) of each are not eligible to enter or win. Void where prohibited.
3. DRAWING: All Internet entries must be received by Pharmavite by 11:59:59 PM on 04/01/08 Pacific Standard Time (PST) as specified above, and all mail-in entries must be postmarked by 4/01/08 and received by Pharmavite on 04/15/08, or they will be void. One (1) prize winner will be selected in a random drawing to be conducted on 04/29/08 by an independent judging organization whose decisions shall be final on all Sweepstakes matters. Winner will be notified by e-mail or via U.S. mail on or about fourteen (14) days following the drawing.
4. PRIZE: The winner will receive five pre-paid gift cards in the amount of $500.00 each, for a total of $2,500.00, which can be used towards the purchase of gasoline. The gift cards are good for one (1) year from date of issue. Odds of winning depend on the number of entries received. Unclaimed prize will not be awarded. Prize is non-transferable and cannot be redeemed for cash, except at Pharmavite’s sole discretion. Pharmavite reserves the right to substitute prize of equal or greater value.
5. MISCELLANEOUS: Winner is responsible for all local, state, and federal taxes. Winner consents to Pharmavite LLC's and its authorized agents’ use of his/her name, likeness, picture, biographical information or voice for any advertising, trade or promotional purpose, in any and all media now known or hereafter devised worldwide (including on the internet) without further consent, review or compensation, except where prohibited by law. Winner may be required to complete, execute and return an affidavit of eligibility, liability release, and where lawful a publicity release, within the time specified by Pharmavite LLC. If the winner fails to complete, sign and return the document as required, or is found to be ineligible or not in compliance with these Official Rules, he/she will be disqualified and the prize forfeited. If a prize or prize notification is returned as undeliverable, the prize will be forfeited. By participating in the Sweepstakes, entrants agree TO RELEASE, INDEMNIFY AND HOLD HARMLESS PHARMAVITE LLC, RITE AID CORPORATION, THEIR RESPECTIVE EMPLOYEES AND AGENTS, AND THE SWEEPSTAKES PARTIES FROM ANY AND ALL LIABILITIES, INJURIES, LOSSES OR DAMAGES OF ANY KIND (INCLUDING, WITHOUT LIMITATION, DEATH OR PERSONAL INJURY TO PERSONS OR PROPERTY) RESULTING (DIRECTLY OR INDIRECTLY) FROM PARTICIPATION IN THE SWEEPSTAKES AND/OR ACCEPTANCE, POSSESSION, USE OR MISUSE OF ANY PRIZE. PHARMAVITE LLC MAKES NO WARRANTY, REPRESENTATION, OR GUARANTEE, EXPRESS OR IMPLIED, IN FACT OR IN LAW, WHETHER NOW KNOWN OR HEREAFTER ENACTED, RELATIVE TO ANY PRIZE, INCLUDING, WITHOUT LIMITATION, QUALITY, MERCHANTABILITY, AND/OR FITNESS FOR A PARTICULAR PURPOSE. Participants agree that Pharmavite LLC, Rite Aid Corporation and their respective agents and the Sweepstakes Parties are not responsible for any technical error, which may occur in processing, collection, storage or distribution of Sweepstakes entries or other Sweepstakes materials or errors, typos, or misprints in these Official Rules or in any Sweepstakes-related advertisements or other materials, whether caused by Pharmavite LLC, Rite Aid Corporation, their respective agents, the entrant, or by any human error. Participants also agree that Pharmavite LLC, Rite Aid Corporation and their respective agents and the Sweepstakes Parties are not responsible for lost, stolen, incomplete, damaged, late, mutilated misdirected, failed, or garbled entries, mail or computer transmissions, or for technical failures of any kind, including but not limited to, electronic malfunction or damage to any network, hardware or software. If the Sweepstakes is not capable of running as planned for any reason, including tampering, computer virus, unauthorized intervention, fraud, technical failures or any other causes beyond the control of Pharmavite LLC or Rite Aid Corporation that corrupts or impacts its administration, security, fairness, integrity or proper conduct, Pharmavite LLC reserves the right to cancel, modify or suspend the Sweepstakes. Pharmavite LLC, Rite Aid Corporation and their respective agents and the Sweepstakes Parties are not responsible for any problems or technical malfunction of any telephone network or lines, computer online systems, servers or providers, computer equipment, or software, on account of technical problems or traffic congestion on the Internet or at any website, or any combination thereof, including any injury or damage to participant's or any other person's computer related to or resulting from participation in this promotion. Any attempt by any person to damage any website or undermine legitimate operation of the Sweepstakes is a violation of criminal and civil laws and should such an attempt be made, Pharmavite LLC reserves the right to disqualify and seek damages from any such individual to the fullest extent permitted by law. The Sweepstakes is governed by U.S. law and is subject to all applicable federal, state and local laws.
